All medical records are considered confidential. If submitting a copy of your medical records to us, please mail or hand-deliver those to: Western Montana Clinic Medical Records P.O. Box 7609 Missoula, MT 59807-7609 Patients who wish to request a copy of their medical records must sign an Authorization for Release of Medical Information stating the reason for the request and what person/agency is to receive the information. Release forms take 7 - 10 business days to be processed. Information pertaining to alcohol or other drug dependencies and HIV test results require a release specific to that information.
